Root cause analysis (RCA) in healthcare is generally defined as a failure analysis occurring after patient harm or near-patient harm has occurred. Apparent cause analysis (ACA) is generally applied as failure analysis for undesired outcomes that didn't harm or reach the patient. Think of the approach as a good catch with a designed review of why it happened. Are you performing reviews of your failures or just moving on?
